IF YOU ARE TAKING YOUR EXAMS ON THE BSU CAMPUS, DO NOT FILL OUT THIS FORM. Proctored testing at BSU is in 202 Decker Hall. Call 218-755-2038 to schedule. Or submit the form to the Advising Success Center if you are taking your exams on the BSU campus.

Other proctor options are public school officials or counselors, librarians, or certain supervisory professionals. Please read the Proctored Exams Guide and FAQ

 Note that many testing centers are at limited capacity for in-person proctoring this fall. Please check with your instructor(s) to be certain your exams require in-person proctoring, or if a remote option is available.

    • Coworkers, friends, parents, or other relatives may not be used as proctors. We will verify your relationship.
    • Do not submit the name of your proctor without his/her permission.
    • It is your responsibility to contact your proctor or test center to schedule your exams.
  • If the number has an extension, please add it behind the position/job title of the proctor in the field above.
  • List the class subject and number for each class requiring proctored exams. Example: HLTH 3300. Use the + icon to add more classes as needed.
