Digital Corps job request form

Please use this form to request job from the Digital Corps

We need to know what to call your project when discussing it. Try to keep it under four words maximum.
Approval
Every job is different, and it is your responsibility to find out what kind of approvals you need to have this job done. At the very least we recommend informing and checking with your department chair, dean, and Office of Communications and Marketing. Have you looked into the necessary approvals, and do you take responsibility for us moving ahead on the project? (note: we cannot work on your project without you getting proper approvals)
Name, Image, Likeness Approval
Does your job require obtaining the approval to use someone's name, image, or likeness publicly? (note: nearly all BSU students sign a waiver before registration, so they are pre-approved)
Leave blank if you do not have a Digital Corps Student Worker you would like to work on your project. (There is no guarantee this student will be able to be assigned to your project):
Please list all stakeholders (can be specific people, or general types) in order of relevance, with short descriptions of their motivations and reasons for being interested in your product. (Add contact info when appropriate.)
Describe what will be the final form of your project (e.g. "poster", "brochure", "t-shirt", "Social Media Post", etc.). If you have multiple copies or parts to your project, please list them all separated by commas.
Include all relevant measurements. (e.g. Aspect ratio and time duration for videos. Bleed, trim, safe area, etc. for printed items, etc.) You can leave this blank if you upload templates in with the proper dimensions in the "Resource Link" below.
Please provide a OneDrive Link with resources (clearly labeled and organized into folders). Be sure to set permission settings to "anyone with the link can view": See this link for the settings: https://bit.ly/3eGkEFy These include any templates, thumbnail sketches, reference images, or other resources to help in the production of this project. (The more of these resources you provide, the more the priority status of your project increases.)
Please enter a desired delivery date for a first draft. Set this date a week or two before the actual due date (Expiration Date), for revisions, edits, and corrections.
MM slash DD slash YYYY
When would it be too late to complete this project (after revisions)?
MM slash DD slash YYYY
Do you need recommendations or references for printing, publishing, or production services? If so, what kind? Keep in mind the Digital Corps is only designing your project. In nearly all cases, you will be responsible to find and hire a printing service that can meet your printing and publishing needs.
